The Hospice Nurse Practitioner works in collaboration with the Medical Director and Hospice Team to perform home visits/evaluations, in-patient evaluations, and/or face to face evaluations as needed. Responsibilities for Hospice Nurse Practitioner Conducts Face to Face visits as required by hospice regulations Performs clinical care including bedside evaluations of patients, history and physicals, discharge orders, patient education and instruction, and participation in case management as needed Orders, performs, and interprets laboratory and radiology tests as needed Prescribes medications including controlled substances to the extent delegated and licensed Orders treatments and durable medical equipment as indicated Provides nursing services in accordance with the plan of care and utilizing the nursing processes. Clinical notes and assessments are completed at point of care and documentation is left in patients chart. Will follow Documentation policy. Requirements for Hospice Nurse Practitioner Must be a licensed Nurse Practitioner in the State of Nebraska or compact state to work in the State of Nebraska. Graduate from accredited school of Nursing. Master degree in appropriate field of nursing. At least two years of experience in hospice, home health, or community health in the past five years.
