Job Description:

Responsibility: Assess the nutritional needs of patients across various age groups and health conditions. Develop, implement, and monitor individualized nutrition care plans in collaboration with interdisciplinary teams. Educate patients and their families on dietary strategies for disease prevention, management, and wellness. Provide medical nutrition therapy (MNT) for conditions such as diabetes, cardiovascular disease, gastrointestinal disorders, obesity, and renal disease. Document nutrition assessments, interventions, and outcomes accurately in electronic health records (EHR). Stay updated with evidence-based practices and participate in quality improvement initiatives. Conduct nutrition-related presentations or group education as needed within the clinic or community.
