Job Description:
POSITION SUMMARY: Identifies personal and medically related
social service needs of the resident and assures that the
individual needs are met by coordinating services with appropriate
disciplines. Makes appointments and provides transportation to meet
the medical and personal needs of the resident. Documents
assessments, care plans and social service interventions per
facility policy as appropriate. GENERAL REQUIREMENTS: To perform
this job successfully, an individual must be able to perform the

requirements listed below are representative of the general
knowledge, skill, and / or ability required: • Knowledge of the
structure and content of the English language including the meaning
and spelling of words, rules of composition, and grammar. • Ability
to use mathematics to solve problems. • Ability to understand
written sentences and paragraphs in work related documents. •
Ability to communicate in writing as appropriate for the needs of
the audience. • Ability to talk with others to convey information
effectively. • Ability to use logic and reasoning to identify the
strengths and weaknesses of alternative solutions, conclusions or
approaches to problems. • Ability to understand the implications of
new information for both current and future problem-solving and
decision-making. • Possess active listening skills to give full
attention to what other people are saying, taking time to
understand the points being made, asking questions as appropriate,
and not interrupting at inappropriate times. • Ability to select
and use training/instructional methods and procedures appropriate
for the situation when learning or teaching new things. • Ability
to monitor/assess performance of yourself, other individuals, or
organization to make improvements or take corrective action.
