Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Shift:/n/n8 Hour Days; no weekends or holidays /n /nWork Experience:/n/n /n/t - The Physical Therapist (PT) is a professional who is responsible for assessing, planning, implementing, and evaluating patient care. /n/t - The PT is ultimately responsible for implementing and documentation of interventions, outcomes, discharge plans and patient/family/care giver education. /n/t - He/she collaborates with physicians and other members of the interdisciplinary health care team to provide continuity of care. /n/t - The Physical Therapist is responsible for the supervision of and delegation of tasks to appropriate individuals including Physical Therapist Assistants (PTA), technicians, students, and other customers of the organizations. /n/t - The PT must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. /n/t - He/she reports to the Director of Rehabilitation Services /n /n/n /nEducation:/n/nGraduate of a professional physical therapist education program accredited by the Commission of Accreditation in Physical Therapy Education (CAPTE)./n/nBachelor’s Degree - Required /n /nLicense:/n/nCurrent Physical Therapist license from the State of Nebraska/n/n /n /nCertifications: /nBLS/CPR Certification./n/nCrisis Prevention Institute (CPI) obtained within six full months from date of hire (only for staff scheduled in Home Health and Hospice)/n/n ProKatchers Job ID 78962.
