Job Title: Home Health Occupational Therapist Job Responsibilities: Conduct initial assessments/re-assessments and develop individualized care plans for clients Provide skilled occupational therapy to clients in their homes, including therapeutic interventions and goals, home exercise program, etc. Coordinate with other members of the care team to ensure comprehensive and integrated care for clients Educate clients and their families on proper self-care techniques and health maintenance strategies Monitor and document client progress and communicate any changes or concerns to the physician Act as an advocate for clients and ensure their needs are met in a timely and respectful manner Adhere to all company policies, procedures, and regulatory requirements Job Qualifications: Active OT license in the state of Nebraska Minimum of 2 years of occupational therapy experience, preferably in home health or a similar setting Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and collaboratively as part of a team Compassionate and patient-centered approach to care Company Overview: Freedom in Home Services is committed to providing exceptional care and support to individuals in the Omaha community.
